Yay, I just got a call from the IRS!

Actually, they were returning my call about "What do I *do* with this info?" when discussing the permatemp payment I got. Apparently, I'm not the only one (by far) who was confused by this, as it's a *really* unique situation. But, I got the skinny, and am now ready to get my taxes done.
maybe this weekend, and get it out of the way.

You need Schedule D.
On line 8:
column a- write in "Viscaino vs. Microsoft"
Column B- for date write in 1999
Column c- that value should be on your 1099-B that came with your payment
Column d- also should be on 1099b
column e- Enter $0
Column f- repeat value entered in column d

finish form as appropriate for your individual situation.

Oh, and the W2 is added into your other W2's, and the 1099-INT is entered on line 8A (taxable interest)
